  • Athletic Forms and Fees

    This is important information if your student is going to participate in athletics at Pewaukee High School, and is ESPECIALLY timely if they are participating in a fall sport.  Below is a list of forms that are required to be on file in the High School Athletic Department PRIOR to the first practice.  It is HIGHLY encouraged that all forms be turned in as soon as possible.  They can be mailed to the high school or dropped off in the main office, in an envelope clearly marked Attn: Athletic Department. 

    Current Physical Athletes are required to have sports physicals on file with the Athletic Department prior to the first practice.  Failure to do this results in ineligibility to participate in try-outs, practices or games.

    Sports physicals (green cards) are required by the WIAA every two school years.  Physicals taken after April 1 are valid for the following two school years; physicals taken before April 1 are valid only for the remainder of that school year and the following school year.  In the second school year that the physical is valid, a tan alternate year card is required.  This updates personal insurance and physician information and gives permission for the student to participate.  The tan card only requires a parent’s signature. 

    The Athletic Department does not work in conjunction with youth sports, camps, etc. so DO NOT assume we have copies of physicals provided for those purposes.  Please call or email Karie Williams in the Athletic Department, willkar@pewaukeeschools.org or 262-695-5073 if you would like to verify there is a current sports physical on file.

    Blank physical cards can be obtained from the hanging file outside of the Athletic Dept. around the corner from the main office.

    Athletic Code, Eligibility and Drug Testing Form:  This form needs to be completed by the athlete and parent to acknowledge WIAA eligibility requirements, the PHS Athletic/Activity Code of Conduct and Consent to Random Drug Testing

    Athletic/Activity Handbook:  Co-curricular code of conduct for all high school athletics and extra-curricular activities.  (This is your copy of the handbook; you are signing acknowledgement of it on the form above).

    Concussion Info Acknowledgement:  State mandated form required from each athlete once per school year. To be signed by parent and athlete before 1st practice.

    Athletic Fee:   The $50 athletic fee is due before the first game.  If you choose to pay at the time you submit forms, please BE SURE the check is stapled or clipped to the paperwork to ensure it doesn’t get separated.  Checks should be made out to Pewaukee High School.  Checks submitted for sports that require try-outs are held until the teams are formed in the event the check needs to be returned.

    Copies of forms can be located below or in the hanging file outside of the Athletic Secretary's office.

    Additional information will be available at the mandatory All Sports meeting scheduled for 6:00 PM on Sunday, July 30th at PHS.   All forms should be turned in prior to this meeting.  This will be an opportunity to meet with individual team coaches, obtain schedules and ask questions.

  • Pewaukee Pirate Athletic/Activity Philosophy
    Pewaukee High School co-curricular activities are a very important component of Pewaukee’s curriculum. While, first and foremost, we want to provide a sound academic experience for our students, extra curricular activities continue to provide special opportunities to promote their mental, physical, social and emotional development. We at Pewaukee High School, feel these contributions to the overall development of our young men and women is essential and we appreciate the support of our programs by our student body, teachers, coaches, advisors, parents and community. The cooperative efforts of the people mentioned above make the Pirate experience second to none.

    We offer the following Interscholastic Sports to all students in grades 9-12. Everyone is encouraged to participate and those that choose not to try out are encouraged to be loyal supporters for all the teams.
